The second opportunity to make the diagnosis and expedite angiography was missed because the ECG never met STEMI criteria and continued to be labeled ‘normal.’ But this third opportunity to make the diagnosis was missed, with an ECG labeled and confirmed as ‘normal’. Delayed angiogram found a 95% mid RCA occlusion that was stented.
