Regular readers know the deal about medical studies. In the simplest sense, you randomize patients to two treatments. Random assignment (mostly) balances the known and unknown factors. You declare an endpoint before the experiment and then report that endpoint. There may be secondary endpoints, but the focus is always on the primary endpoint.
