Cricothyrotomy is a rare procedure used in the “can’t intubate, can’t oxygenate, can’t ventilate” scenario. Last, and most important, is decreasing pulmonary vascular resistance. For most of us, this will be with IV tPA, but one should also consider nitric oxide, catheter directed tPA, or ECMO (extracorporeal membrane oxygenation).
