The company has now posted results of the EPIC-SR trial. In EPIC-SR, the same drug had no significant effects. The negative results of EPIC SR can be explained in the same way as the ICD story. Nothing has changed with the drug; its chemical structure remains. What has changed is the clinical situation.

Left Bundle Branch Block

ECG Guru

We see a SR with broad QRS complexes (more than 120 ms). Why is this a left bundle branch block? These are positive in the leftward leads I and aVL and in V5 and V6. In the more rightward leads III, aVR, and V1-V3, the QRS complexes are predominantly negative with deep S waves. This is a typical LBBB pattern.

In the third line you see a short SVT over 3 beats, then SR again. Initially there is an irregular fast pulse without P waves, which corresponds to a tachyarrhythmia in atrial fibrillation. At the end of the first line, after a very short pause, there is sinus rhythm. The QRS complex is widened to over 120 ms.

Right Bundle Branch Block

We see a SR with broad QRS complexes (more than 120 ms). Why is this a right bundle branch block? These are positive in the rightward leads III and especially in V1 and V2. In the more leftward leads I, aVL, V4-V6 there are clear S waves. This is a typical RBBB pattern.

Sudden SOB - Pulmonary embolism

Wee see a SR with LAFB and conspicuous T-wave inversions in the inferior leads and in V1-V6. ECG 1 is from a 57-year-old male with no prior cardiac disease. He reports acute shortness of breath for 2 days. These are typical ECG changes that may indicate a pulmonary embolism. ECG 2 was taken from the same patient 1 year earlier.
